In Network
We are in network with Aetna, Allegeant, Blue Cross, Blue Shield, Carefirst, Cigna, Medicaid (Carelon), and United Healthcare.
Out of Network
For out of network insurance holders, a super bill can be provided for you to submit to your insurance company for partial reimbursement.
Private Pay
You can pay out of pocket for services rendered. You may choose this option for the purposes of flexibility and privacy. A super bill can be provided upon request.
Forms of Payment
We accept Credit Card (Visa, MasterCard, American Express, Discover). We also accept FSA, HSA, or check. Co-Payments will be collected at time of visits.
